We inquire into the question whether the Aristotelean or classical \emph{ideal} of science has been realised by the Model Revolution, initiated at Stanford University during the 1950ies and spread all around the world of philosophy of science --- \emph{salute} P.\ Suppes. The guiding principle of the Model Revolution is: \emph{a scientific theory is a set of structures in the domain of discourse of axiomatic set-theory}, characterised by a set-theoretical predicate. We expound some critical reflections on the Model Revolution; the conclusions (...) will be that the philosophical problem of what a \emph{scientific theory} is has \emph{not} been solved yet --- \emph{pace} P.\ Suppes. While reflecting critically on the Model Revolution, we also explore a proposal of how to complete the Revolution and briefly address the intertwined subject of \emph{scientific representation}, which has come to occupy center stage in philosophy of science over the past decade. In order to cure the social sciences of many of their inveterate diseases, it seems to me extremely important and useful to support and develop in some measure A.S. Tsipko's ideas on the doctrinal preconditions of our difficulties and failures in the building of socialism, ideas that have provoked such wide-ranging responses.

Objective: To assess the adequacy of the process of informed consent for surgical patients at the University Hospital of the West Indies. Method: The study is a prospective, cross-sectional, descriptive study. 210 patients at the University Hospital of the West Indies were interviewed using a standardised investigator-administered questionnaire, developed by the authors, after obtaining witnessed, informed consent for participation in the study. Data were analysed using SPSS V.12 for Windows. Results: Of the patients, 39.4% were male. Of the surgical procedures, (...) 68.6% were scheduled, 7.6% urgent and 23.8% emergency, 35.2% were minor and 64.8% major. Information imparted/received was acceptable in 40% of cases, good in 24% and inadequate (unacceptable) in 36% of cases. Almost all (97.6%) patients stated that they understood why an operation was planned and 93.3% thought that they had given informed consent. Most (95.2%) thought that they had free choice and made up their own mind. A quarter (25.2%) of all patients were told that it was mandatory for them to sign the form. There was a discussion of possible side effects and complications in 56.7% of patients. Conclusions: This study clearly indicates that surgical patients at the University Hospital of the West Indies feel that they have given informed consent. However, it also suggests that more information should be given to patients for consent to be truly informed. (shrink)

The interaction between cavity modes and optical transitions leads to new coupled light-matter states in which the energy is periodically exchanged between the matter states and the optical mode. Here we present experimental evidence of optical strong coupling between modes of individual sub-wavelength metamaterial nanocavities and engineered optical transitions in semiconductor heterostructures. We show that this behaviour is generic by extending the results from the mid-infrared to the near-infrared. Using mid-infrared structures, we demonstrate that the light-matter coupling occurs at the (...) single resonator level and with extremely small interaction volumes. We calculate a mode volume of 4.9 × 10 from which we infer that only ~2,400 electrons per resonator participate in this energy exchange process. (shrink)

B. R. Frieden uses a single procedure, called extreme physical information, with the aim of deriving 'most known physics, from statistical mechanics and thermodynamics to quantum mechanics, the Einstein field equations and quantum gravity'. His method, which is based on Fisher information, is given a detailed exposition in this book, and we attempt to assess the extent to which he succeeds in his task.

The editors planned the appearance of this article to coincide with the seventy-fifth birthday of the author, which was to have been marked in December. The issue had already been approved for publication when the sad news came of the sudden death of Aleksandr Fedorovich Shishkin, prominent Soviet philosopher, member of the editorial board of our journal, a remarkable human being, our comrade and friend.

Nicholas Wolterstorff's Divine Discourse attempts to give philosophical warrant to the claim that ‘God speaks’. While Wolterstorff's argument depends largely on his appropriation of J.L. Austin's speech act theory, he also uses two narratives that for him demonstrate how ‘God speaks’. The first is the story of Augustine's conversion in the Confessions and the second is a story that Wolterstorff recounts about a certain ‘Virginia’. This study argues that what Wolterstorff claims to derive from Augustine's narrative for his view of (...) divine discourse is not fully supported by the Confessions, and that Augustine's interpretation of the words ‘tolle lege, tolle lege’, can be construed as a useful interpretation of an ambiguous sign. This is consistent with Augustine's understanding of the interpretation of texts in both the De doctrina christiana and the Confessions. In short, Augustine is far more open to the ambiguity of signs than Wolterstorffs's account suggests. (shrink)

The problem of the psychological unconscious has a history of many centuries. Some investigators seek to demonstrate the existence of the unconscious, and others deny it. Fundamental differences exist with respect to the nature of the psychological unconscious even among proponents of that hypothesis. In recent decades interest in the problem of the unconscious has grown substantially in our country. An extensive literature devoted to this subject has appeared in which discussion centers principally on the significance of the psychological unconscious (...) in various spheres of human activity. However, little attention is given in those works to making clear the specifics, the patterns, and the forms in which the unconscious is manifested. Virtually nothing is said about the techniques by which we penetrate the realm of the psychological unconscious, how we achieve knowledge of it, and what the psychological relationship is between the consciousness and the unconscious. In this paper, it is explained that a dialogical approach to complex argumentation can be fruitful for solving two important problems concerning the analysis of the argumentation structure. First, such an approach makes it possible to clarify the distinction between coordinative and multiple argumentation structures, and to identify clues in the presentation for each of these structures. Second, a dialogical approach can provide a basis for dealing more adequately with refutations of counterarguments.

The paper discusses Dr. Floris Tomasini's paper “What Is Bioethics: Notes toward a New Approach?”. Based on Tomasini's account of methodological and ethical pluralism, the paper explores the demarcation problem of bioethics and suggests a full methodological laissez-faire.

This addendum non-trivially strengthens one of the six claims of my two-part paper 'The Equivalence Myth of Quantum Mechanics', published in this journal in 1997.

OBJECTIVES: To study some ethical problems created by accession of a previously nomadic and traditional society to modern invasive medicine, by assessment of physicians' attitudes towards sharing information and decision-making with patients in the setting of a serious illness. DESIGN: Self-completion questionnaire administered in 1993. SETTING: Riyadh, Jeddah, and Buraidah, three of the largest cities in Saudi Arabia. SURVEY SAMPLE: Senior and junior physicians from departments of internal medicine and critical care in six hospitals in the above cities. RESULTS: A (...) total of 249 physicians participated in the study. Less than half indicated they provided information on diagnosis and prognosis of serious illnesses all the time. Physicians who were more senior and those who spoke Arabic fared better than other groups. The majority preferred to discuss information with close relatives rather than patients, even when the patients were mentally competent. Most of the physicians felt patients had the right to refuse a specific treatment modality, and 68% denied patients the right to demand such a treatment if considered futile. Further analysis showed that physicians' attitudes varied along a spectrum from passive to paternalistic with the largest group in a balanced position. CONCLUSIONS: In traditional societies where physicians are regarded as figures of authority and family ties are important, there is a considerable shift of access to information and decision-making from patients to their physicians and relatives in a manner that threatens patients' autonomy. Ethical principles, wider availability of invasive medical technology and a rise in public awareness dictate an attitude change. (shrink)
